What is a Geriatric Care Manager?
 
 
Katherine Lerner,
who is a Social Worker and a Geriatric Care Manager, describes it this way:
 
A Geriatric Care Manager assesses, plans and coordinates services being provided for your loved one, either at home or in a care facility. Identifies your senior's unique care needs and formulates a care plan. Helps with Out of Home Placement, if required, helping you locate an appropriate living situation and helping you with the transition. Assists you in understanding the benefits and coverages of your Insurance Plans. Provides information on the various community services available for seniors and assists you with the application process. Navigates the Complex Social Service Bureaucracy and assists you with the increasingly complex procedures of obtaining eligible services.
